DG17--Primary Care Providers - Everyone Needs One!

Primary Care Providers (PCP) play a vital role in diagnosing and caring for patients with Cystic Fibrosis. Pediatricians are the first introduction to a medical home and are often instrumental in helping the family come to terms with a CF diagnosis. However, as the family is introduced to the CF care team, many neglect the primary care model and rely heavily on the CF team for primary care needs. It is also estimated that by 2025, 70% of CF patients in the US will be cared for by adult providers. It is imperative that they have a dedicated PCP that will manage their non-CF health outcomes and implement preventative care measures. Preventive maintenance by a PCP span from childhood to adulthood. In this session, we will look at an overview of primary care maintenance for young and aging adults with CF i.e: cancer screening, dyslipidemia, hypertension, mental health, pregnancy and immunizations. We will look at strategies to partner with PCPs for management of CF and the educational efforts needed to engage and empower PCPs of patients with CF.

  • Discuss preventative care measures for CF patients that primary care medical homes can provide.
  • Describe novel care models collaborations with CF team and primary care providers.
