As many people with CF (PwCF) are experiencing improved health outcomes and improved nutrition due to highly effective modulators (HEMT) and advancement of other therapies, management of GI, nutritional and endocrine co-morbidities pose new challenges. While some individuals continue having difficulty maintaining optimal nutritional states, others face challenges of overnutrition, and complications associated with obesity. As overnutrition is becoming more common place in PwCF, higher incidence of hypertension and hyperlipidemia are also emerging. In addition to individually tailoring our approach to nutritional counseling and management to match patients’ nutritional goals, additional data is needed to evaluate if overnutrition and diabetes in PwCF is associated with increased risk of comorbidity, and micro and macrovascular complications, as it is in the general population. Further, additional studies are needed to evaluate risks vs benefits of potent pharmacotherapy to treat overnutrition and/or diabetes, such as GLP-1 receptor/GIP agonists.

Learning Objectives:

To employ individualized strategies to optimize nutritional outcomes that will benefit both underweight and overweight individuals and help PwCF achieve their nutritional goals.
To present treatment approach to overnutrition, ranging from lifestyle optimization and medical-nutritional therapy, to pharmacotherapy to surgical weight loss approaches.
To discuss the impact of obesity on CF-related diabetes.
