Sarah Kubiak, PharmD, BCPS, received her Doctor of Pharmacy degree from the Bouvé College of Health Sciences at Northeastern University in 2006 and worked as an inpatient clinical pharmacist at Boston Children’s Hospital (BCH) for the following 15 years. Throughout that time, she provided inpatient clinical pharmacy services to the pediatric and young adult cystic fibrosis teams as well as the infectious diseases consult services at BCH. In 2022, the Cystic Fibrosis Foundation awarded Sarah and the BCH CF Team a grant for implementation of outpatient clinical pharmacy services. With the help of this grant and the opening of BCH Specialty Pharmacy, Sarah was able to establish the role of an “all access” clinical pharmacist for the adult and pediatric CF teams at Boston Children’s Hospital and Brigham and Women’s Hospital combined programs. She transitioned fully to this role in September of 2023, and now helps to manage patients' CF medication therapies under a collaborative drug therapy management agreement. Alongside a dedicated pulmonary pharmacy technician, she provides outpatient and inpatient clinical pharmacy services to over 600 patients with CF and also provides medication counseling for patients with CF who utilize BCH Specialty Pharmacy. Sarah’s areas of interest include infectious diseases, therapeutic drug monitoring, and pharmacogenomics. She lives near the start line of the Boston Marathon in Hopkinton, Massachusetts with her husband and 3 children, and enjoys cooking, hiking, and snowboarding with her family during her free time.
