Patient transitions of care can lead to increased patient adverse events. Healthcare team recognition of transitions and timely communication between all involved parties is imperative. Pharmacy care team members play an essential role in CF patient transitions of care. This session will discuss innovative ways to seamlessly provide care.

Learning Objectives:

Discuss important transitions of care that occur over the CF patient lifetime.
Formulate strategies to improve communication and patient safety across patient care transitions.
Describe strategies to assist with medication access during transitions of care.
